Peoples Health Selects eQHealth

eQHealth has been selected by Peoples Health Network of Louisiana to provide training support to PHN's team members who provide assistance to hospitalized members. This care coordination program is expected to reduce unnecessary hospital readmissions by improved coordination of care.
